Another Patients radiology report is in MyChart

I was on my fiancés patient portal (MyChart) to see if his cat scan images or reports were uploaded yet because we forgot to get the disc back from the office. Instead they scanned someone else’s radiology report into his chart by mistake. I don’t want to get anyone in trouble for a mistake but his report could be in the other patients chart and we need access to them. Who do I contact about this?
