Why does my medication cost $8000 in the US and $20 in Mexico?

My insurance in the US wouldn't cover Alinia (nitazoxanide), an anti-parasite medication, since I need it for SIBO which is an off label use. It costs $8000 out of pocket. my doctor said in Canada it costs $500 out of pocket. I am in Mexico currently and I got this drug under the brand name Rosanil for $20 at the pharmacy without a prescription.

Why do the prices differ so wildly?

Local Dr. office is only accepting appointments for annual physicals, not sickness.

After a large Atlanta hospital system bought our small family practice years ago, logistics and communications have constantly deteriorated. Their medical staff is still very good and who I want to see. I was just told that they don't accept quick appointments any longer for currently sick patients. (earliest available was February, when I'll be dead if not treated.) WTF !!! Only option now is to visit an Urgent Care facility, which generally doesn't have MDs, but PAs or NPs at best.

Recently saw a specialist who proceeded to write me 3 prescriptions and send me home with an armful of med samples. Thought it would be interesting to see how much money they take from pharmaceutical companies in comparison to two of my non-pill pushing physicians.

Which countries have mandatory return of service for medical graduates?

I am not talking about certain schemes whereby a government or organisation fund your medical school (often +/- a bursary) in return for you working in a disadvantaged area for a while.

Are there any countries where (nationally) medical graduates have to work in that country for a few years in order to pay back the government for investing in their training, as it were. It comes up for discussion now and again in my country, and is wildly unpopular as a suggestion for obvious reasons. I was wondering if it's in place anywhere else.
