ethical question

If a patient went to a doctor for a check up for a heart condition and reported symptoms like chest pains and heart palpitations, and the doctor said the patient should go to a hospital, so the patient agreed and promised to go as soon as he left. But he didn't go and ended up having a heart attack later. So he spent a week in the hospital and left when he was finally stable. He already had a follow-up appointment scheduled with the same doctor the week after he got out. However, the doctor refuses to see him because he did not go to the hospital like he said he would. Is this ethical of the doctor?
