Reading doctors’ notes and their frustration with insurance

Recently noticed a small shift when reviewing orders and notes before I see patients. Maybe it’s just an EMR change but reasons for orders and consequences of not doing the recommended action seem more blunt, and appear to be getting things done. The more academic, medical theory language is left for the narrative sections. Example 1: Consequences of not: Death Example 2: Delay in authorization has caused significant decline. Patient now has x that may be permanent, stop them from walking, and require additional surgery. (Adjusted to protect)
