SAFE reports being abused

My wife is an OB Hospitalist and has managed to find a hospital that seems to encourage tattling. SAFE reports have been written about her by nurses she works with that states “she didn’t look at me right” or “she was a little mean”. Now, I’m not in the medical field, I come from an oil field background where safety is no joke, so I completely understand the purpose they serve and their importance. However, I can’t help but feel like this is getting out of hand. So I was just curious if any doctors out there have experienced such abuse and how they handled it. Thank you in advance.
