Buy & Bill Margin Erosion

Question I wanted to get some insight on. I am an accountant who works in healthcare, and spend a lot of time analyzing drug margins. I am trying to better understand the WHY behind why margins are eroding in recent years (from provider perspective). I’ve done research and it seems like a few big contributing factors are 340B and Medicaid but not sure if I fully understand why. In the case of 340B, creating lower ASP due to the discounted price they are receiving thus driving ASP and reimbursement down in totality or is that not what’s happening there? Any insight would be appreciated here
