How to get my cheek birthmark (Melanocytic nevus) removal covered by insurance at Kaiser in US

I've talked to my doctor and the dermatologists at Kaiser numerous times and had pictures sent to dermatology numerous times, but they always say it's benign and I gotta have the plastic surgeon out of pocket. Any tips?
