Covid paperwork

Since the CDC has decided we are able to return to work 5 days after testing positive, my job now requires me to provide a work release explaining why I was out sick for six days and not five. I don't have health insurance or a PCP. They have a self-test and a rapid test from Walgreens that shows I was testing positive. I'm not sure what to do because the 6th day I missed was Easter (a fireable offense). Are there doctors that would allow me to just come in and have them fill this out? I can't afford much because I'm having to do all this for approved UNPAID time off. Just not sure what to do, I called and the company wasn't much help at all and just kept telling me to have a doctor fill out the medical forms. Would I be able to just take this to Walgreens and have the pharmacist fill it out, is that legal?
