Healthcare for LGBTQ+ Community

Hey everybody! I am an undergraduate student working on my senior research project and we are sending out a survey that focuses on healthcare for the LGBTQ+ community. The purpose of the survey is to evaluate the care rendered to individuals within the LGBTQ+ community in healthcare facilities with religious affiliation. If you feel comfortable, please fill out this ANONYMOUS survey about your experiences with healthcare.
