Can one go about finding out information & a potential negligence a year later from a hospital? If so, how?

If a person passes away in the hospital, can the victim's family a year later ask for an explanation as to what happened if the explanations of the doctors does not seem to add up? Especially if the family feels there's a strong chance of negligence?
