[Question - Other] Medical Chart Questions

• In the US, does every provider/facility have their own charts? Or is it central data storage, so if you change providers/facilities, does it "move with you"?
• What is the legal source for the patient's right to get the chart corrected? I keep pointing out a glaring error on a daily basis, she refuses to correct it. Are there deadlines I can insist on? Would a new provider start a new chart or keep adding to the old one? Is this state law or federal law?
