Doctor late with disability paperwork

I’ve been seeing a doctor and discussed taking a medical leave with him. Even booked an appointment to help him answer questions on the paperwork. I have not gotten the paperwork back and have tried contacting the office but seem to be getting the run around. He originally said he’d probably have the paperwork finished for me a couple days ago. Called today and told them if I didn’t have that paperwork in a two days, I could potentially lose my job since I’ve been out on leave and we had the deadline set by my employer. Has anyone have a doctor just ghost them regarding paperwork even when you went to an appointment to fill it out and you had agreed upon the dates and deadline? I’m hoping to get answers and my paperwork on time, but I’m really concerned about my job security and what to do if I don’t receive the paperwork.
