New potential employer requires "ADULT HEALTH APPRAISAL FOR CHILD CARE". It's a form that has 3 boxes. Two say I can physically do the job, and the third says I don't show visible signs of TB. Requires doctor signature, and not sure if I'll need a TB screening or what.
Where should I go to get this done? I don't have insurance, don't have a doctor, and want to spend as little as possible because I'm in debt and have no savings. What's the lowest cost option? Anybody know how I get this thing filled out?
