UHC: is this fraud?

This new dr I'm seeing refuses to give me a superbill and made me pay in cash. She said since shes' in network with UHC but she's choosing to not accept new UHC patients (I'm on PPO choice) she cannot give me a superbill . I've never heard of this. Is she allowed to do this? thanks
