Is it a trend for insurers to deny colonoscopies as close to the procedure time as possible, after the patient has already started liquid diet/drinking the "prep" solution, so they won't want to fight back about self-pay?

Wonder if anyone has documented this. Have heard of procedures getting denied very last minute.
