Can people outside of the US really not go into medical bankruptcy even with private healthcare systems and medical tourism?

I realize that going bankrupt due to medical tourism isn't exactly the same as medical bankruptcy in the US, but I keep seeing people say that it doesn't happen elsewhere when I know medical tourism is huge in some parts of the world. Many countries also have private healthcare systems alongside the public, so it just seems like there is absolutely a possibility of small number of people going medically bankrupt in countries with universal healthcare. For the record, I am 100% in favor of universal healthcare, but I'm a healthcare expert and don't like internet inaccuracies that seem to be made up just to create a meme.
