Giving Birth Outside of USA?

I’ve been curious lately while on my journey to become pregnant and wondering if anyone knows the answer to this: If a US citizen gives birth in a country with free/low cost health care will they still have to pay the huge sums that the US asks for, or will they get the initial birth and first few days of care for free? ie, if I travelled to Canada the week of my birth and used the health care services there, would I only have to pay for the lower cost of healthcare or would the US try to collect money still? (this does not include postpartum health care such as checkups and vaccines etc) From the small (and not super helpful) but of research i’ve done so far, I understand that a passport would be needed for the infant after the birth but other than that chaos, it seems like traveling abroad for childbirth has some great financial benefits…
