Do doctors get in trouble from negative feedback on surveys?

I recently switched back to a doctor I had previously seen for a chronic condition. I requested an appointment and the scheduling experience was really bad. They took weeks to respond, and when they finally offered an appointment time, I replied that I wanted it but they never booked it and didn't respond to my requests to confirm it. Then they finally called to apologize and offered a same-day appointment.

The appointment was more three weeks ago. This doctor took over management of my medication. When I saw her, she said she would increase the dose and send it to the pharmacy. When this hadn't been done after a week, I messaged the office and asked to get it done before I ran out. When they didn't respond after a week, I called. They said they would mark it urgent. Now it's been five more days and it's still not done and I will be out after tomorrow.

Now I have a survey in the mail and am not sure how to respond. This has been a shitty experience but I also like this doctor so much better than my last one. I'm worried if I fill out the survey truthfully that she'll get in some kind of trouble, but I'm also frustrated at how bad the whole thing has been. She is part of a large medical system in my city.
