Bill adjustment removed after 10 months

10 months ago I received a bill for $250. I called my doctor's office, and an adjustment was made for $250, making my balance $0. I was told today that I owe $250. I called the phone number on the bill and was told that I owe $0 by the robot lady. I spoke to a representative who said I owe $0 but that the adjustment had been denied and then had been made again today. I am very confused. How does an adjustment get denied? I'm worried it will be denied again.
