Help on where to start with ankle surgery

So I have a hdhp and an HSA with a good chunk squirreled away. I'm relatively young (32) and have severe ankle instability. I am searching for a new pcp physician because my last one was pretty terrible. My question is, where do I start with trying to get ankle surgery? The goal is to have this procedure go towards my deductible, I just don't know how to get the procedure approved. Would I need a referral from a PCP or could I go to a specialist first? Is there a way to see if insurance would consider this towards my deductible, it's just all so confusing and I need to get this right. None of this is on my medical record btw as I've been hesitant to create some sort of prior condition history or get dinged on health insurance premiums. Thanks in advance.
