Has it always been typical for earliest possible appointments to be several weeks out from when you call to schedule, or is this a side effect of the pandemic?

I didn't start going to doctors / scheduling my own appointments regularly until the last year or so, and I'm still baffled by how long it takes to see anybody. A couple instances it took me almost a month to see my primary care, only to have them refer me to a specialist, and by the time I saw the specialist it had been nearly two months since I first noticed symptoms. I feel like I'm constantly playing catch-up with doctors. I've even asked to be put on wait lists for if earlier slots open up, but they never seem to. Has this always been the way it's been? Is it dependent on which health services / campuses I use?
