[Question - Insurance] What can I do about this?

Earlier this year I received a COVID test at my local hospital because I was showing symptoms. I was on my family's insurance plan through my mother's employer at the time. (Anthem Blue Cross Blue Shield)

A month later, my mother left her job and thus didn't have health insurance anymore. 2 months later, I receive a bill from the hospital for $400! I also receive a statement from BCBS saying that they didn't cover $400 of the cost of the visit. The hospital is in network, why didn't they cover the cost of the test in full?

There was no due date on the bill so I did not pay it right away. 3 months later the hospital send me the same bill. I plan on disputing it, but I don't have access to my old insurance card and don't know my plan number or anything. What should I do?
