[Question - Other] In USA, what documents should you start gathering to apply for a sliding fee scale? Didn't make income this year, Mom paying for online school tuition, living w/ GF and paying half the food w/ savings

The only income I made this year was from some interest from some certificates of deposits. There is no W2 available for that available yet but I could print off some informal statements from Marcus Goldman Sachs, which I got the CDs with. I'm staying with GF right now in her apartment. She makes money but I don't. I just don't have any documents.

Where can I get documents I need to apply for this and how would you go about getting them?

What are some common things that could stop me from getting approved for a sliding fee scale at a community health care center to see a doctor?
