It seems obvious to me that economic incentives are aligned for payers to penalize unvaccinated and reward vaccinated members through things like premium discounts or other measures. I think I have seen maybe one instance of something like that happening. I am wondering whether there might be legal or contractual barriers to it.
I would fully expect some states to react to such measures with legislation to ban that type of reasonable incentive though.
