[Question - Insurance] Is this insurance fraud?

My dentist told I might have TMJ disorder few months back and referred me to their partner clinic. It doesn't pain and I never felt the need to see a doc but lately has been an inconvenience/irritating as it produces cracking sound. Could be due to lot of stress (death in family and other issues) probably so I consulted the referred clinic.

I consulted a doctor who suggested a jaw splint while sleeping and told some manual exercises. It felt like they indirectly forced me into it as I felt it was not necessary. While on the dental chair only, I told them I need to check with insurance and they said they have already connected with my insurance and I need to pay only 10% after deductible so we can start right now and built it taking my measurements. It costed me $550 after insurance paid.

Fast forward now (second visit), I went to take the splint and it's too tight, the doctor tried to adjust it for almost 50 mins and due to continuous trials I started having tooth ache and gave up. I said I can try at home since I wasn't even sure if it was fitting due to ache. I tried at home and it immediately started paining canines and a bit in Incisors. Moreover during this (second) visit I told the doctor that even after manual exercises, it's same, she didn't even pay attention.

Are they doing insurance fraud but since I sort of agreed (forced or didn't deny) in the first visit, I am not sure? It seems they were only interested in selling the splint.
