[Question - Insurance]

Does anyone have the NYS Empire Plan that includes United Healthcare? If so, could your please help me understand the co-pays? I may need a diagnostic colonoscopy. Would this be just the outpatient surgery copay or would I have to meet my deductible first? Tried calling them and they were not helpful.
