How can I help my family deal with my morbidly obese aunt?

My dad's sister is morbidly obese. She's in her mid-late 60's and not looking good. She is probably in the 400-450 pound range and a smoker. This renders her essentially immobile, making it very difficult to get her to a primary care doctor (which she has been without for years). The primary care doctors she has seen have been reluctant to take her on as a patient, according to her. She lives in Oregon, but my parents live in Idaho, making it essentially impossible to help her. Recently the power went out and without her powered recliner, she was incapable of getting up. Obviously, this is of grave concern to us and we are trying to figure out how to help her. We don't have experience dealing with obesity in my family and are having a really hard time finding information on google. This is my longshot.

My question is the following: What is the best path forward? Is our best option to try and find a general practitioner who will make house calls who can recommend her to a specialist? Do we sort of have an "intervention" and enroll her in some sort of adult fat camp? Any advice would be appreciated. Let me know if there is information I am omitting that would be useful.
