Out of Pocket Maximum Vs. Total Maximum out of pocket

So I was checking my benefits online, and it said my Out of Pocket Maximum is $1500, but then that my Total Maximum out of pocket is $4000. Also, my deductible is $1000.

I understand the difference between deductible and max out of pocket. But Why are there two of them?

These are both In-Network charges... the out of network also has those three components at different amounts.

Any ideas?

edit1: I have Highmark PPO

am beginning to think this is an ER Out-of-pocket-max????

p.s. Sort out your health insurance system America!
