Wrongly diagnosed
Long story short I had a provider diagnose me with Mild Alcohol Abuse in Feb 2022 at the Veteran Affairs. I recently found out about it when I did a flight physical and it was brought up to me.
The problem is, I've never been remotely closed to diagnosed with something like this and after reviewing the clinic note itself, I believe the provider typed in someone else's visit into my chart because some of the items discussed weren't even from our visit.
The provider no longer works at this clinic and I'm meeting with another one Friday to discuss. I was told it would be an easy fix but it seems to good to be true.
How do I go about getting this off my record and corrected?
