What does “first visit” mean?

When looking at summaries of healthcare plans for next year I’ve seen some state that “primary care visits $30 copay ( first visit for either pcp, mh, sa, or sp before DD)” does this mean they only cover the first visit or is the copay only for the first visit or something else entirely?
