Last month I ended up at an urgent care for bronchitis that was not improving. Today I logged into my patient portal (because I have yet to receive a bill) and looked at the record from the visit. The visit notes start off with:
“(Name) is a (#) year old female with a history of seizures currently taking (specific medication).”
I do not have seizures, nor have I ever had a seizure in my life. The specific medication that is mentioned is one I have been on for several years. Apparently it’s main use is to treat seizures but I have been prescribed it by a few different psychiatrists over the years as a mood stabilizer. None of this was discussed with the provider I saw, they just asked what medications I was on.
I’ve heard horror stories of people with inaccurate info in their medical records causing issues down the line. I guess I’m paranoid that because some doctor stated at one point that I have a history of seizures when I definitely do not that it will be treated as factual. My question is, would this be something I should try and get corrected? Is a visit note a permanent piece of my medical history that other providers can see and possibly impact future care? This urgent care was affiliated with a major hospital system in my area through which I see multiple providers and all my records are together in the portal.
