Is this normal behavior for family practice doctor in the US for referrals?

I have an HMO insurance plan in the US. I went in to my family practice doctor 9 months ago because of a bout of lower back pain, hoping to get a referral to a spine doctor. Did my annual physical at the same time. He gave me a referral to some quack with terrible reviews who works in a solo practice, not part of an orthopedics group or anything. Didn't feel comfortable going to that guy. I never made an appointment, pain went away, moved on. Now 9 months later I'm having severe problems again. Call up my doctor to see if he can change the referral to point to a doctor in an orthopedics group I went to some years ago for a different issue. His office tells me they won't change it and that I have to schedule another in person appointment with my family practice doctor to get the referral changed.

Is this normal? This is a massive waste of time in my view. Do doctors give referrals over the phone normally? Especially if they just gave a referral for the same reason 9 months ago? I'm getting ready to just switch doctors because of this but not sure if it's just normal course of business.
