What happens if you are hospitalized with Part A but no Part B

I am a U.S. citizen residing in the Philippines. I dropped Medicare Part B as no benefits are paid overseas. Now I may need complex cervical spine surgery in the U.S. So, there will be initial exams, MRIs. These will not be covered. The surgery and hospitalization (hospital, anestheology, surgeon's fee) will be covered under Part A? Rehab and follow-up will not be covered? Comments please! Perhaps it would be cheaper to go to Europe or Singapore and pay the whole thing out of pocket than to go to the U.S. with only partial coverage. Your recommendations?
