Getting a cortisone shot without insurance?

Hi, I am "between insurances" and need to get a cortisone shot in my subtalar joint. I can get a referral/prescription from my previous doctor, and have all my images. Every place I've called so far wants me to make an expensive appointment to see a doctor first. All I need is the shot. I'm in the NYC area but am in Philadelphia often and can travel. Any ideas for who, or what kind of place I could contact to get the shot that won't cost me a fortune?
