My pharmacy shorted one of my refills?

I picked up a refill of my Lorazepam yesterday (15) and only just now noticed that there’s only 10 pills in the bottle - and someone had written “only 10” on the label.

Is this normal? Did they just not have enough on hand to fill the entire prescription? Never encountered this before.

Process to get a vasectomy

I'm not asking about the actual surgery or details like that. I'm more so wondering about everything prior. Do you just sign up for one and then have one done, or is there any information they try to get you to know prior? What degree do medical practitioners ensure that the patient knows what they're getting into? Is it consistently that way or is it doctor to doctor/ hospital to hospital?

I was working overseas on a work assignment for 5 years. Towards the end of my duration I became very ill and ended up in a foreign hospital. I should have had the surgery there. I returned to the US and felt better, but I was misdiagnosed while out of the US. I continued working remotely in the US (away from home) and had to drive myself to the ER. I drove myself in the company vehicle to the hospital, throwing up several times along the way. I had to save money and not pay for an ambulance.

ON TOP OF ALL THIS, I now have to pay a $5 service fee for every payment I make on this.

Context, I have had insurance my entire life, paying into time after time again. Thousands of dollars every year. I never had to use it until now... All this time paying into this system, and now I HAVE A $5 SERVICE CHARGE ON MY PAYMENTS.

Provider Overbilled, now I have too much in FSA to cary over

My provider overbilled me for a procedure at the end of 2024, which is confusing as they requested a pre-authorization from Insurance. Insurance said they would only cover a portion of the cost, so I payed the balance out of pocket and planned to get reimbursed by FSA. It's now past the new year and my EOB came back stating that Insuramce was covering the whole amount (minus a copay). It's nice that it was covered, but I also planned on that cost being deducted from FSA so I didn't use it on other things, now Ill have too much in my FSA to roll over the entire amount. Am I out of luck? Is there any way to get all of it rolled over?

When i finish training or sleep on back and breath on nose i feel pain in heart

Hard trainings and eating a lot should give you a lot of condition and healthy body but for me only pain in heart and i must stay and breath heavier and not move for it to stop.

Lets not talk about stuffs that i train for years and eating a lot is healthy and good for me because i was anorexic before and im 28 male.

So maybe i have nose deviation because its little curved and breathing while im sick and had cold is impossible. When i sleep on left side my heart also gives me pain. Had sleep paralysis all my life and maybe it made wounds inside my chest or heart. I dont eat sugars food or drink and drinking protein with milk and vitamins magnesium+calcium. Also i felt this pain while i used hot showers and had less air to breath

applying for covered california but it seems super scammy- what am i doing wrong?

hi everyone! i’m just reaching out for help. i’m a college student applying for health insurance all on my own for the first time.

i reached out to covered california to get info on insurance. i made an account on their site but after entering all my personal details, it said it couldn’t verify my identity. it gave me a number to call, who then told me i needed to meet someone in person to confirm my identity.

i call this random guy, who texts me to meet him at an address today at this time. he says he has to see my ID and social security card. well, after calling these people i keep getting several calls from people asking me if i know about all the details of my plan. after chatting, they hang up suddenly.

when i call back, the numbers are disconnected. i’m sooooo confused. is this real? has anyone gone thru this process?

How to take time off to recover with no sick leave or insurance.

I hope this is okay to post here, feel free to remove otherwise. Hi, thanks in advance and I hope I don’t sound too mopey. I have a connective tissue disorder that has caused major issues with my wrists, hands and knees ongoing for six months. Knees are swollen and difficult to walk on making me nervous as I work a hard labor job. I’ve been trying to carry on as usual but it’s definitely not having a good impact on my health and I worry I could end up disabled and unable to work if I don’t do something soon. I’m just trying to think of a way I can take some time to recover so I don’t end up with permanent damage. I’ve no insurance or chance of paid sick leave. I’m wondering if anyone knows of any programs or something that might help. I’m thinking of asking my employer to “fire” me so I can qualify for unemployment and get medicaid but maybe there’s a better way. Thank you.

An "in network" provider. One bill "out of network".

It was late at night in June and I rolled my ankle. I wasn't sure if I broke it. I have anthem insurance. I live in Indiana and I was visiting the state of michigan for one month. Now you have the backstory.

I went to an Orthopedics medical office that was listed as in network on my insurance website the next morning. They had an appointment open and it turned out that I had a fracture. Every bill was covered. I paid the rest of my deductible and what I owed the office whenever a bill came. The office visit and x rays were covered. For some reason they billed my crutches and brace separately a few months later out as of network.

Anthem insists that the office is sending the bill to the wrong address and it should be covered. The provider insists that that are sending it to the correct address. They claim that it was sent to the same place as the other bills that were covered. I feel like I shouldn't have to pay an additional $400.00 because they can't figure this out. The bill has been submitted 4 times. I've called both places 5 times. The billing person from the orthopedic office doesn't answer when I call with an anthem advocate. They will call me back days later. Anthem will put me on hold and leave a message and ask the provider to submit the bill again. There is no follow up. I just get another EOB that says the provider is out of network. They are both blaming each other.

Finally Anthem filed a grievance for me today. But, Anthem said I probably won't hear anything. What does that mean?

Any advice? Has this happened to anyone else with one bill from an in network provider?

Medicaid (2025)

Id like to have a genuine debate regarding the current climate of healthcare reform. Medicaid expansion could possibly be targeted but I have my doubts.Id like to remain as politically centered as possible.

Should the administration slash funding for Medicaid, Millions of people would be without healthcare institanously. This would be in due part of the 9 states who have trigger laws. There are multiple issues with this beyond the obvious.

  1. All those people would now be eligible for special enrollment periods resulting a flooding of the market. They simply would not be able to handle such an influx of people.

  2. The cost of healthcare would increase significantly. This is simply supply and demand. Millions of people would no longer be visiting doctors or avoiding hospitals like the plague. This would drive hospitals and physicians to charge more for services to make up for the lack of business that Medicaid brings. This would also cause private health insurance plans to skyrocket as the insurance companies are going to have to renegotiate their payouts to providers.

  3. The task of disenrolling millions of people would also result in the loss of jobs of thousands of benefit workers at social services. The cost of administrative fixes to hundreds of state laws regarding Medicaid expansion would need corrected and it won't be cheap.

4.If you believe that this administration is truly trying to find ways to decrease the deficit and help the people, then you would need to agree that cutting Medicaid funding would be against their agenda.

  1. If you believe that this administration is trying to line their pockets at the cost of the American people, you would also have to agree that the insurance companies as well as the hospital networks are at an enormous loss and the conspiracy that all these powerful people in healthcare have any say in things falls short.

Everything seems contradictory to itself. The only rational thing in my mind seems that it's in the administrations best to keep funding Medicaid for multiple reasons. Are there places within Medicaid that can be cut? Sure. I can think of a few that would have minimal impact to the functioning of society while saving millions. But to remove expansion benefits entirely seems foolhardy at best. I don't believe they would cut funding altogether but perhaps may reduce the percentage they fund they states over a set period of time. And should things end up so bad the Republicans would lose the election in 4 years and the Dems would have an enormous leg to stand on.

Again, I'd love to see some varying points of view while maintaining a rational mindset. I get that political stance is a factor but and opinions on the president vary but I genuinely would like to know what people think is truly going to happen in the coming months.

Commercial Insurance vs. Medicare Advantage??

Looking online and can find a ton of resources explaining the difference between traditional Medicare and Medicare advantage, but would anyone be able to explain the difference between commercial insurance and Medicare advantage? All I know to this point is that Medicare advantage plans are offered thru private commercial insurers (I think?) but I can’t seem to find much else out there. Would greatly appreciate the help!

How do hospitals in the US determine the markups they charge on prescription drugs?

I am specifically looking at expensive treatments like cancer drugs (keytruda and similar) but any info is helpful.

Do they just charge a % over the cost they acquire the drugs, so the more expensive the drug the more expensive the markup?

Or is it based on the actual dollar cost of administrating the drugs, so more expensive drugs will have the same dollar amount markup as cheaper drugs if they have similar administrative costs/difficulty?

Need Email for Cigna Dept that Handles Policy Disputes

Even though I have CIGNA Medicare supplement G for which they are withdrawing from my account and have presented me with a welcome packet and a card the same insurance that is listed on my Medicare profile. No one at cigna Customer Service or Cigna IT can find my Medicare supplement contract number. Which means as far as they’re concerned, I don’t have a Medicare supplement. I can’t register my cna.com because prior to turning 65. I had an individual plan if I try to register because I have to use my name my phone number my Social Security they say I already have an account if I try to use my Medicare supplement number they say no number is found.

I’m pretty sure that this is some sort of weird oh, you didn’t use your middle initial Policy thing or as someone suggested some angry broker deleted my account because I had gone ahead and not chose their medical advantage option (whatever is left after ruling out other options, no matter how unlikely, is the answer 🤣) but I’ve documented 9 hours on the phone with customer service/IT and have tried to sign up for my cigna 57 times.

It appears the cigna Medicare department and the record-keeping have been severed (Medicare lot from cigna keeps telling me to sign up at my cigna) and I feel I just need to talk to one intelligent empowered individual dual to sort this out.

However, given that I have invested days of my life into trying to solve this absurd problem, and given that this problem has impeded my ability to access the services contractually obligating Cigna to provide for me. My next step is regular regulatory boards.

Denied adequate pain relief.

(PS, yes, this a bit of a long post) Yes, I am an addict who, under the correct circumstances and with adequate pain relief would completely abstain from all illicit substances the moment a PM contract is signed by me. Im hardly a full time addict like you'd think I am or the context of being an addict suggests. I may use once a month if I can afford it.

The only things that have ever worked for my pain, is of course, opioids. I am on 120mg of methadone under the ideology that 'one or two sizes fits all addicts' approach of medication assisted treatment. Unfortunately, for the addict whose in immense pain and is most likely an addict due to the neglect of pain relief in healthcare. Had I been adequately treated for my pain and not sent home with nothing for after care pain relief, I would have likely never been an addict and had to self medicate and seek my own treatment.

Now, the thing is, I would get clean prior to walking into a PM clinic as well as taper my methadone to a dose low enough to be switched over to the other opioids that I know to completely decimate my pain compared to the half assed pain relief methadone provides or the placebo effect buprenorphine has upon my pain. They both are rather insufficient for my extent of pain.

I would jump through all the hoops and do everything needed until they figured out an managed my pain. I've been through a long list of non-narcotic options and the only thing that's ever touched my pain was full agonist opioids aside from methadone. It seems healthcare wants me to rather suffer instead of actually having my quality of life and relief from pain. Im tired of being bed ridden in pain even on a high dose of methadone, it's terribly obvious due to my continued use on methadone that I do not get enough pain relief from the methadone alone. The only time I use otherwise is when the first line of defense being marijuana doesn't quite work then I have to resort to illicit street opioids to do the work methadone isn't really doing whatsoever.

Not treating me even though I'm ready to do all the right things and jump through all the hoops is bullshit. It's not any different than not treating someone for their ethnicity. They cannot change their ethnicity the same way I cannot change my past, but I can dictate how my future goes except for having adequate pain relief. It seems doctors who are not me and do not feel my pain have all the authority to dictate someone else's pain, that they do not feel, is not severe enough to warrant opioid narcotics.

Im not drug seeking. I dont want to get high. I haven't been able to get high in 5 years or so since my wreck which essentially made me bed ridden and disabled me. There is no high worth forfeiting my ability to obtain adequate healthcare. I want the lowest dose possible to be able to manage my pain, you don't hear addicts asking for the lowest dose possible let alone anything about pain. I also can't tell doctors what medications have worked the best and what medication do not work for shit otherwise I get labeled as drug seeking.

Id like to prove myself and show that I am no longer a liability, but no one will ever give me that chance it seems. To never give me that chance is unethical, inhumane and honestly bullshit healthcare. No patient is undeserving of adequate pain relief except for me apparently. I know there is greater extents of pain relief available but I'm forbidden from them and stuck with a life of pain. If doctors are unethical enough to leave me in pain the rest of my life, will they atleast be unethical enough to euthanize me since I'm tired of this life of being bed ridden and immense pain. Could they atleart do me that favor? Anyways, if I sign a DNR, it's only me and my doctor that need to sign it, no one else correct?

i live in Nyc and have Molina affinity. i keep hearing bad things about them and was thinking of changing on Monday to Healthfirst.

So many places don’t accept it anymore, but I’ve been with them for years. I received a letter of dismissal from them regarding a call that never happened. I’m considering switching to Healthfirst Medicare, but I’d like to know your thoughts. Also I got a letter from nyc insurance marketplace that I was auto renewed and wanna know if I can't anymore?

Unable to access my FSA money

Not sure if this is the correct subreddit for my issue.

I maxed out my 2024 FSA to cover six months of physical therapy after surgery, but I’m unable to access all the funds because my physical therapy provider is not providing an itemized bill that includes all charges.

From the beginning of my treatment, this particular PT group has operated differently than most. On day one, I signed a form agreeing to a $20 facility fee added to "certain insurance visits" to cover rising rent, qualified staff, equipment, etc. Additionally, they require a credit card on file, which is immediately charged for the co-pay ($75 per visit) plus this $20 facility fee, bringing the total to $95 per visit. This arrangement seemed reasonable at the time and didn’t raise any red flags. I planned to use my FSA to cover the entire cost.

At the end of the year, I requested a superbill and submitted the required documentation to my FSA. The PT provider gave me a patient ledger showing the $75 co-pay but omitted the $20 facility fee per visit. They also provided a credit card receipt showing the total charge of $95 ($75 co-pay + $20 facility fee) per visit and the general form I signed regarding the $20 facility fee.

Because the $20 facility fee was not listed on the patient ledger, my FSA denied reimbursement for that amount. I went back to my PT provider asking for additional documentation to support this charge, but they refused. They claim that the $20 facility fee is “not considered medical” and should not be documented in the patient ledger. I’ve explained that I understand it’s not a medical cost but I should still be provided an itemized documentation for this fee. They continue to refuse to provide any further documentation.

This involves approximately 40 visits I had with this provider last year. The $20 fee per visit amounts to $800 that I am unable to get reimbursed due to the lack of proper documentation.

Can the PT group to refuse to document this charge? My FSA administrator suspects fraud and that they are pocketing this cost without declaring it.

**A side note, they also overcharged me on my credit card one day, and they refuse to reimburse me the overcharge until I am "discharged and all claims and reimbursements are processed". Can they do that? **

I am happy with my therapist, the treatment and outcome, and it is cheaper than other providers in the area. I am just so sketched out with the billing practices lack of transparency and asking for advice of how I can get my FSA money?

Recently turned 18, all of a sudden have an old medical debt?

Recently turned 18 and all of a sudden I’m getting letters and the occasional voicemail that there’s like $30 of medical debt from like ~middle may.. I’m covered by my parents’ insurance and regardless, that should’ve already been handled and paid for during whatever clinic trip it must’ve been.

It’s a bit irritating and concerning that I’m getting “debt collectors” calling me for $30. Both my parents and I are confused as to what it’s even for/from.

So, kind of a vague question, but any insight on what this may be and what action to take?

any other options I'm missing?

hey everybody. tl;dr is that I don't have health insurance because I think it's fucking ridiculous to pay upwards of $200 a month for shit I don't even use. but I know it's dangerous not to have any, so I'm looking for other options.

the cheapest option with my employer is about $270 a month which is more than I'm willing to pay. but since my employer offers "affordable" health insurance I'm not eligible for the tax credit that discounts your monthly premium thru the marketplace. my question is if all of the health plans that exist are listed on the marketplace? i.e. if it's even worth it to try to explore outside of the options marketplace shows me. because if marketplace shows literally everything that exists, I don't have to dig any deeper cuz the cheapest thing on marketplace is $320 a month. I've tried looking at a couple specific insurance company websites but I don't know if that will possibly yield different results than what marketplace tells me. I'm new to looking for secondary options because in the past, my premiums buying healthcare thru the marketplace have been <$20.

Prior authorization needed after changing insurance plan

Hi, so I’ve gotten myself into a situation and just needed advice on the quickest way to solve/get through this. For context I have tricare prime, I’m a T1D and have been seeing a specialist for awhile. I recently switched from tricare select to prime bc the additional cost for almost the same plan did not benefit me. I also just switched over to a new PCM bc my other one was out of network. I got a call from my specialist saying I needed prior authorization due to the switch in plans, even tho I’m still apart of tricare and I’ve been a patient with them for over a year. I understand having to do this but the issue comes in with getting an appointment with my PCM bc they go out a couple months and I need medication before then. The specialist had me call my PCM and ask if they’d just send over the authorization without seeing me, which I knew was a long shot. So what would be the best course of action to get this done asap bc I have a limited supply of medication. I understand these are the hoops I have to jump I just need to get through them quickly bc I need to medication to live.

Hospital consolidation is NOT working.

I feel hospital consolidation simply leads to a monopoly which allows them to raise prices on nearly every service . I think it harms consumers. The vertical integration which allows hospitals to employ every single doctor that works there violates some anti-kickback statute im sure and is against the corporate practice of medicine. In short, I think its a wrong move. We need to go back where all doctors are solo practitioners in pvt practice competing against each other. and more independent hospitals competing against other hospitals. As is right now, there is no incentive for hospitals to improve 8 hour wait times in the ED if they have a monopoly on healthcare.

Is being a health care assistant worth it?

Hi so here’s the background to what’s been going on. Since 2007, I’ve been in retail, customer service for a while and for a 5 years been also working at jobs as a cleaner. Right now I’m working as a key holder at Dollarama for $18.05 an hour. I want to get away from the retail world due to dealing with shoplifters. I’m an introvert but would love to help people. For awhile I’ve been working with a case worker at WorkBc and and she mention that I work become an early childhood education assistant or could look into the option of taking part of a HCAP program. Where I become a health care worker also they pay you go to school because it’s high demand. My case worker says I could receive $29 an hour. I’m not sure if I should take it. Have any of you guys are health care assistance and is it worth it?

How come PSWs/recreationists make so little money compared to "easier" jobs??

I recently started working in a dementia day care program. My offical title is a recreationist but I toilet these people, I make food for them, I feed them, I run programs, I clean, do laundry...you name it. I make 21 bucks an hour. So do the PSWs I work with. And I am basically also doing their job as they only come in at night. My bf has a job at a hospital, that consists of cleaning machines but he basically gets up to 4 hours of doing nothing during his shifts (its a very laid back job) and he makes 34 bucks an hour. He doesn't interact with patients and his only job is to set up/ clean machines. He always tells me how nice it is to have such an easy job but to make such good money. I'm happy for him but feeling a little frustrated that after 4 months, I am already burnt out, have experienced horror stories and see little compensation for the mental and physical work I'm doing. I've been bitten, hit, sexually assaulted and pissed on, i had poop all over myself etc. Now, I am resilient and I don't mind none of that- but I do mind the lack of pay.

I looked into how much PSWs make and it's laughable. Out of everyone in healthcare, we probably make the least amount even tho we deal with EVERYTHING. I'm at a loss. Something needs to change. Anyhow, I am going to change professions probably sooner than later.

I live in a dystopia

Im 29. I use a rollator to walk. I have spondylitis and fibromyalgia. Walking can be agony. It's 9 degrees out. I force myself to get my ADHD medication so I can work. I'm on my third insurance in the past six months. I was originally on my own, then switched to my husbands once i was eligible, then his company switched to a new one. I get to the CVS pharmacy and they say its $260....with insurance. he works for a forbes 500 company. the insurance has CVS Caremark whatever that means. They say I need to pay $260/month until I hit the deductible in which case it 'should' be lower. I don't have that amount of money on me. I need to walk back home to get some cash and then come back. I'm broke until my student loans post. I'm going to need to switch back to my private insurance which was $280 a month. I'm in so much pain I don't know if I'll make it back today. I'm so tired of this.

This likely seems like small potatoes..but its just one of a hundred heartbreaks I've had with healthcare this year, and my standards were low to begin with.
