Seeking professionals willing to share their experience in a few different roles

Hello! I’m excited to finish my undergraduate degree, but I’m just beginning to picture my life in the workforce and it’s daunting.

I have mentored professionals in the roles of community (government/non-profit) educator and (long-term care) administrator. Additionally, my mother works as an RN in a hospice setting.

I’m looking for some insight into a few professional spaces within healthcare:

-LMHC or psychologist (if your clients refer to you as a “therapist” I would love to hear from you!)

-Nurse Practitioner

-(LTC) Administrator or dir. of nursing

-Director of hospice; adult day; etc

-Any related roles! If you do case management, community Ed, or a social work-related profession, please share! (I know there’s a separate sub for this but if you stumbled along my post, I’d appreciate any input!!)

Anyway, any general insight would be much appreciated!!

If a prompt would be helpful: Do you like your role? What is most challenging/rewarding? Do you plan on staying in said role? How is work/life balance? What advice would give young professionals with an interest in doing similar work?

Healthcare.gov won’t let me change my email

The first time I signed up for insurance through them I was using an old email address that I now use for spam. When I called customer service in December they told me I cannot change my email address associated with my account. I currently go into my old email and have all healthcare-related stuff automatically forwarded to my new one but I really want to close that email account entirely so I wish they would just let me change it.

EDIT: When I log into my account and try to change it myself it just gives me an error message and says the changes did not save.

Can\should I report a former mental health service provider for medical insurance fraud?

Hello, I have a question about a former mental health service provider I use to see a nurse practitioner at and if what they are doing qualifies as fraud and is of what would be the best way to report them to get them off my back.

I started working with this group and meeting with a nurse practitioner virtually who gave me anti-anxiety medicine over the summer. During this time, I asked the group multiple times how much I would be expected to pay for services and was never once given a clear answer for months. Only after I asked my health insurance company did I get a straight answer and found out how much I was being charge for 10 minute zoom calls ($150 a session). Granted a lot of that surprise was my own fault as I should have done more to understand my family health insurance plan first.

Once I realized how high the price was I reached out to the group to understand the pricing and see if I could negotiate a lower price through a personal payment plan. At first customer and billing service were very friendly and let me know they would have the owner contact me soon to figure out a reduced price. Instead, no one from billing or the owner has reached out to me in the five months since, despite my repeated calls and communications with the company.

Then the company slapped all of these charges onto my credit card. At this point I had stopped seeing the nurse practitioner but I still had a almost $500 charge on my credit card. In December I contested the claim through my credit card company because the group refused to communicate with me and it felt like they were just trying to wait me out to pay it on my credit card before it started affecting my credit. After some back and forth the credit card company ruled in my favor in February and I thought that was the last I would hear from the company.

Instead a week ago I get an email from the company telling me to pay a bill through their online portal. The bill is less than the one originally charged to my credit card but still a high amount. Since then I have called and emailed the office multiple times trying to talk to someone about this new bill but have yet to receive a response. I was able to talk to someone at the front desk last week who said they would get someone in touch but still no response and now even when I call the front desk it goes straight to an answering machine. I have looked up online reviews of this group and my experiences are far from unique. Lot of people talk about poor service and no communication and receiving unexplained bills on their credit card a year after stopping services with the group.

Based off these experiences I have a couple questions. Is what i have described above count as any form of fraud which I can seek legal redress for (or at least get them to drop the charge) and if so what would be the best way to do so? If not then given that this charge is not on my credit card what would be the consequences for not paying it ?

How does OxyContin work?

Hi, I am a business student doing research on the opioid epidemic, but I am having trouble (surprisingly) trying to find a difference between OxyContin and generic opioids. As far as I could tell, OxyContin is a controlled-release pill while the generics are usually not. What does this mean, and is it possible to explain it in the simplest way possible? Thank you!

Johns Hopkins University Press published my book on a new health care reform proposal today

The book first explains why the current system cannot be “fixed.” Then it describes an innovative approach to transform it to an evidence-based system, offering universal coverage and universal access, that is insulated from political and financial pressures.

The proposal relies on three innovations: an independent National Medical Board (based on the Federal Reserve Board of Governors) that will oversee the entire health care system (public and private), a Health Information Platform (HIP) that will unify all medical data under one server (overseen by the National Medical Board) and an evidence based Tiered Benefits (insurance) System that is based on traditional Medicare/Medigap. There are many benefits for this system including benefits for patients, clinicians, public health, the economy, businesses, government and even private insurance firms. Finally, there is an easy ask from Congress- create a National Medical Board the way Congress created the Federal Reserve in 1913 (no need for complicated legislation like the ACA and other reform proposals).

Healthcare in the U.S.

Untied States? Land of the free, home of the corrupt? I had an appendectomy. Surgeon messed up and cut a blood vessel with the laparoscopic machine. Stitched me up. My abdomen filled with blood, I told the nurses I was in bad pain. One told me “I’m supposed to be in pain, I just had surgery”. It got so bad I told them to kill me please. That’s when they brought the surgeon back and he got frustrated with them because he said my stomach is not distended like this. Emergency CT showed abdomen full of fluid. Emergency second surgery and blood transfusion. Several apologies after this for not taking me seriously. Then I got an infection at incision site after second surgery. Told them it hurt at the incision site pretty bad. They did a scan, said I was fine. Sent me home. I had red stripes start creeping across my abdomen the next day, and the burning and inflammation at the incision got severe. I called and was told to come to ER. They made me sit in the ER for 3 hours in pain with a blood infection they caused. Then they had to admit me again for several days because they were worried about sepsis. I had several apologies for not listening to me before. Again. The surgeon hit my femoral nerve and caused nerve damage in my leg, and in my abdomen. I had to see a muscle and nerve specialist who told me this is why my right leg is in pain constantly. I haven’t worked in 8 months. I was sent home anemic, the hospital didn’t give me enough blood. I was white and yellow for two months after the procedure. My GP noticed my labs 2 months later and was mad, asked if the hospital even did a transfusion. I was hospitalized for cdiff 3 months later because of the massive amounts of antibiotics they gave me. Hospital bills are over $360,000. Because of one young surgeons mistake. What’s really unfair is the state of Texas capped malpractice lawsuits to $250k, so I can’t even sue because an attorney wants 40% and they said it’s a waste of their time. No attorney will take a malpractice case now in Texas. Nice job Doctor’s and insurance companies for quietly paying your lobbyist to get that law pasted for you. Now you can butcher human beings and have no real accountability or cost for you. Also, insurance companies have clauses that state if you sue and win, they get paid back first all the medical bills. Nice right? Screw this country, this state, and it’s sh!t medical system

Jobs fighting injustice in healthcare?

Hello! I am a college student in the US, and I’m trying to figure out what kind of career path I want to follow. I am extremely passionate about injustice and inequality in healthcare, and there are few things that make me angrier than seeing pharmaceutical companies preying on vulnerable Americans by relying on misinformation to improve their bottom line. Overall, I just take issue with the general prioritization of profits over people’s well-being that occurs so often within our healthcare system. I know that whatever I do with my life, that’s the cause I want to fight for in my career. The issue is, I have no idea what that career would even be. I’ve recently been considering law school, as that seems like a good place to start, but if anyone here knows of any jobs that would allow me to make a difference in this area, I would love to hear them! Remove the uterus

Good Morning, I come to ask for information, already a few years ago, that I have this idea of wanting to remove the uterus, I am 26 years old, close to 27, I have a boyfriend and we do not want to have children. The reason that leads me to make this decision, is because I have horrible pains even horrible menstruation, I get to take 4 pilss to pass the pains, rubber hot water in the belly right on top of the skin, I get red belly, I never got burned, and I cry constantly. How much should an abdominal ultrasound cost?

I recently received my explanation of benefits for a partial abdominal ultrasound. I was expecting to pay about $400 per some quick searches on Google, but the imaging center is charging $1000, and Cigna negotiated it to $800. This seems to be double the national average.

Are there some sites that provide clearer estimates? What's the best way, if any, a lower payment?

Looking for a stop gap between COBRA running out and start of my new benefits with new employer

So I got laid off and my COBRA election deadline came and went. Just started up with a new employer however my health insurance won't kick in until May 1st. So for the rest of March and the beginning of May I am uninsured and it's giving me anxiety, BAD!!!

Research shows that Black patients tend to be undertreated for pain

'Research has shown that, relative to white patients, black patients are less likely to be given pain medications and, if given pain medications, they receive lower quantities (110). For example, in a retrospective study, Todd et al. (10) found that black patients were significantly less likely than white patients to receive analgesics for extremity fractures in the emergency room (57% vs. 74%), despite having similar self-reports of pain. This disparity in pain treatment is true even among young children. For instance, a study of nearly one million children diagnosed with appendicitis revealed that, relative to white patients, black patients were less likely to receive any pain medication for moderate pain and were less likely to receive opioids—the appropriate treatment—for severe pain (6).'

Scheduling Surgery and Insurance

After years of tonsillitis and tonsil stones, my ENT finally decided it’s time for a tonsillectomy, so at the beginning of March we get the process started. Im also in the middle of switching jobs, which means I’ll be losing my insurance at the end of this month.

I was having big anxiety (job switch, impending tonsillectomy, moving in a couple months), and had rapid heart rate and shortness of breath less than a week before I got a call from the anesthesiologist, who gave me screening questions. When I brought up my anxiety attack, they said I had to get it cleared before I could get he surgery. I met with my primary care asap who concluded, it was in fact anxiety, and gave me the green light for the tonsillectomy.

I call the scheduler for the surgery now thinking I’m cleared, but she said she needs to fax them and have them fill out forms, and won’t get back to me next week.

They only do surgery’s on Tuesday’s, and because of the job change, I won’t have insurance after the end of the month.

My main two questions are, do you need to be cleared by insurance when you schedule the surgery or when you actually get it? The next would be, if not, is it unheard of to be scheduled the day before the surgery? Freaking out big time.

Is it possible to have 10% penalty fee waived if I draw funds from my 401K to pay for a hospital bill?

I had an emergency hospital visit and the bill is really high. Im currently unemployed so I'm considering drawing everything from my 401k to pay for a portion of the bill. Is it possible to get the 10% penalty waived? So sorry if this isn't the right sub. Let me know if there's a different sub to post in. Struggling right now

Anyone work for HealthPartners?

Hi! My wife is a non-union discharge planner at a HealthPartners hospital. Every year, she goes in for a performance review and is offered a “raise.” She never attempts to negotiate. Most years, her raise exceeds inflation, so I stay out of her business. However, this year, she was really excited by her 4% raise. I do think it’s bigger than she’s ever received, but I also know that a 4% raise in today’s inflation is actually a significant pay cut. I really think she should have at least asked for a better offer, but I have no idea if that’s how it works in the medical social work field. Would there be any point in her trying to negotiate? She’s part time (.6FTE). She’s been with HealthPartners for at least seven years. She did recently get an increase in FTE this year, so they might have blown her off if she had negotiated, but I am curious if anyone in a similar position in HealthPartners has successfully negotiated a higher raise. Thank you for any advice you all have.

My co-worker's 340b story

One our office worker's explained to me how expensive it is to buy insulin for herself as she has suffered from type 2 diabetes for years. She gets paid hourly and makes minimum wage while supporting herself and her son and sometimes having to work a part time second job working nightshifts as a waitress. A month ago she purchased discounted diabetic insulin through 340B which she never knew she was eligible for in the past. Due to this she does not have to work as many shifts as a waitress and can spend more time with her son and feel less stressed about affording insulin that she needs to survive. I thought it was worth sharing her story of a federal government subsidy drug program that I never heard about before myself, but is clearly making a difference in ordinary people like my co-worker who just want to receive medicine she needs at an affordable price. My hope is someone reads this and can inform their friends or loved ones about programs like these so things can be just a bit easier for the people that need it the most.

My GP scolded me for seeing another doctor when she was busy

I had a health scare and wanted to see a doc while it was in the acute stage. Since my GP was busy, i decided to go to a walk-in clinic and saw a doc . I ended up getting an appt with my GP a couple of days later, and I told her the symptoms and said that I went to a walk-in doctor in the meantime, and she freaked out.

She covered her face and said "Ok I'm going to get a bit bitchy here, my PAY gets DOCKED everytime you see another doctor! You use the healthcare system A LOT (umm, so?) and it affects me when you go see another doctor". Then she spends 5 minutes explaining to me how the walk-in clinic works and how she sometimes works there and next time i should just show up to the walk in clinic. I was just quiet and apologized, saying I didn't know... She then shrugs and says "Well, whatever test you did with that doctor, I will NOT have access to it. So I have no idea, I won't be able to access the results, you'll have to get them from him".

Then she starts giving me a science lesson I didn't ask for about urine culture tests which I just sat through and nodded... and when I asked about my symptoms and the probability of it being serious, she didn't answer the question, but quizzed me. "Why do you think I'm not worried about that. I want YOU to answer that question. Why am I not worried about that??"

it all seemed a bit chaotic, argumentative and condescending. Thoughts? Do you think this was professional?

Also, anyone know if i really dock her pay by going to another doctor? Is that a faux-pas?

PS I live in Canada for anyone wondering, so covered by our Medicare here. I waited almost a decade to get my own GP, and I’ve been her patient for about a year now.

I live in Italy, where the National Health System is progressively falling apart; it used to be a flagship, but years after years it got worse and worse, pushing citizens to pay private medical examinations and treatments. So I was wondering if there’s some sort of “positive” aspect in a fully private healthcare system, based solely on insurances.

What's a good way to find doctors and hospitals in-network?

I've always worked for companies with PPO healthcare plans so it's important to try and find healthcare in-network. My healthcare website (BlueCross/BlueShield) has a way to search for in-network providers but I've noticed it's horribly maintained. I'm fairly sure they outsource the search function as when you click on "search", you're re-directed to a completely different site with a different domain name. Once there, when I search for general doctors, it shows just a few in my area even though I live in a major metropolitan area. Additionally, over the years I've noticed those search results never change even though I'm sure there are new doctors, some have moved away, etc.

Should I just be calling around to see if doctors accept my insurance and are in my network? Are there other sites I can use to find in-network doctors? I can't believe a huge company like BlueCross/BlueShield can't even maintain a up-to-date website of their own with every doctor and hospital that they cover.

[advice] For NY Medicaid, how do I compare plans & should I choose at time of application?

I am filling out the Medicaid application for my in-laws and see that the application asks for a health plan selection, otherwise it will do an automatic enrollment in one of the plans.

I was leaning towards Healthfirst but wanted to make sure their doctors, pharmacy etc was covered & also wanted to check that co-pays aren't too expensive. Wanted advice on how to do this since I'm not getting much info from the customer service reps?

PCP not willing to order lab work

Hello. I visited my PCP for some health issues such as fatigue, headache, weight gain. I also have a visit scheduled at an endocrinologist who asked to get labs done prior to the appointment (since I need to wait over 3 months) and provided me with a list of tests to get. My PCP only ordered some basics such as CMP, CBC, TSH and Urinalysis. He skipped any hormones, detailed thyroid and micronutrients tests which are the key in locating my problem, even when I asked if he would add these and mentioned that TSH alone will not show anything. He would not listen to anything I am saying and was very dismissive. He just said that he knows better and has over 20 years of experience so he doesn't need anyone's opinion and just left the office. I asked the front desk if I can see other provider or if they can ask my doctor to add the other tests, but they said that they will not order labs for any other provider and it will stay in their office. I said thank you and cancelled my next appointment. A few day later I received a letter from that office that they will not be able to treat me anymore and I should remove them as my primary provider.

Is it normal that doctors act this way and sends a letter like that?

Were they right that PCP will not order tests if a specialist asks for it prior to appointment?

I would appreciate your opinion.

When is enough finally enough?

Given the myriad of articles. Workers quitting in healthcare, public discord etc.

When will enough be enough in the United States to establish a single payer system and to rid a whole industry?

Not an act here and an act there. A complete gut and makeover.

Let discuss how this can happen. I think it should alarm everybody no matter who you are that we have medical plans (normal ones) that sell for close to 90,000 USD per year. One should immediately ask how is everybody not paying that can potentially find themselves in a bind.

Have you ever been discriminated against by a healthcare professional?

I am a college senior working on my thesis which focuses on the diagnosis process within the United States and the discrimination and differing standards of care provided. As a part of my additional research, I am looking for first-hand accounts of discrimination as many people are unable to/are not comfortable with reporting healthcare professionals after experiencing this.

If choosing to share your experience, please also include why you were discriminated against (race, gender, sexuality, income, weight, age, etc.) and any info you are comfortable sharing about how these acts affected you (mentally, physically, financially, etc). Most importantly, did you ever receive the care you needed and deserved?

Let’s talk expanded No Surprises Act

Providers have a difficult time telling patients what a test is going to cost due to complex health insurance policies. Got it.

Why is there no mandate that health insurance plans and/or provider pricing must fall into one of a few billing categories, so patients know or can deduce their costs prior to treatment? This way, people can either contact their insurance company to see how the provider cost will get handled, or they can see which category their insurance falls into based on a universal tier system.

Yes, the American healthcare system is potentially fucked up beyond repair, and, sweeping rules like the NSA have passed, so why not something like this? Has this been tried before? What can we do to initiate legislation for this?

i don’t know much about insurance so please bare with me. i’m almost 19 and still live with my parents, I have Horizon NJ Health. I’m trying to go to therapy and do not want my parents to find out. they still pay for my insurance and everything however. so, I wanted to know if my dad(who does everything insurance related) gets some sort of emails or regular mail monthly or early with all the services i’ve used or maybe copayments or whatever it is or is that not a thing. is there anything i can do about it? is there anything else I have to worry about? or can i just find a therapist who takes my insurance and just start going and he would never find out(insurance related wise at least). thank you in advance!

Nausea and retching occurs when brushing teeth.

I started from a week ago, the morning brushing teeth will appear retching nausea phenomenon, but not at night, which makes me feel uncomfortable, I use electric toothbrush, excuse me, anyone know what is the reason, or what is the good solution?

Getting Endocrinologist Appointment Sooner

My doctor referred me to an endocrinologist. The soonest appointment I could get with anyone in the hospital system was not until mid July. I can't wait that long. I need to see someone sooner. How can I get to an endocrinologist sooner in the United States? It's important enough that I don't care about my insurance being accepted or the cost. I can't wait 4 months. Where can you go or who do you pay to get treatment?
