[Allowed Charges] My medical costs suddenly increased and I can't understand why

I saw a therapist for a single session a few months ago and my insurance was able to cover it in full -- I only owed a $20 co-pay. I recently went back to that same therapist and have been seeing her regularly. When the insurance bill came in for those sessions, I was shocked to see that I am responsible for paying $400 now! I tried comparing the explanation of benefits document from a few months ago with the recent one. I am using the same therapist. Her price has only increased by $5 between now and a few months ago. The difference is that my insurance is only covering a fraction of the expense -- the "allowed charges" amount is much lower on the new bill.

Does anybody have an idea why the "allowed charges" changed for the same service? I don't think it has to do with a deductible, especially since I still have the $20 co-pay in addition to the $400 charge. (Side note: can someone confirm that if you do a co-pay, the deductible isn't put into play?)

I'm going to try and call the health insurance company ASAP. Since I'll be traveling for the next 2 weeks, I might not have time to go on multiple calls. Can people help me better understand what's going on so I know how to potentially contest the charges? I'd like to be able to grasp this all and just make 1 or 2 calls instead of several calls while I'm traveling.
